Our online forms collect contact details, the service requested and, for clinic referrals, the sending clinic’s details. We use this information to review your request, contact you or the referring clinic and arrange care.

What to share

Please provide only the details requested on the form. Health card numbers, insurance member numbers, clinical records and payment information are not needed here. We can arrange an appropriate way to obtain further information if necessary.

Who can access requests

Requests are stored using our website hosting provider and are available through an access-restricted clinic inbox. They are not displayed publicly. These forms do not subscribe you to marketing.

Referrals and consent

A referring clinic must confirm the patient agrees to the referral and to being contacted. Patients submitting directly must agree to be contacted about their request.
